My Premise Health is a secure mobile app that puts your health information in your hands. Real-time appointment scheduling, view lab results, check vitals, message your provider, manage medications, conduct video visits, complete forms and connect with Apple HealthKit to share your steps, glucose, Blood pressure, and other values with your provider. With My Premise Health you can: View your health record Book and manage appointments Message your providers and care team View lab and test results View current medications and refill prescriptions Conduct virtual visits Complete forms and eCheck-In View billing history, statements and make payment online View your immunization history and health reminders Manage your dependent’s health information Share HealthKit values with your provider It’s never been easier to get, stay and be well. To use the app and access your health information, you must first create an account through your healthcare provider or have an active My Premise Health account. Don’t have an account yet?
